Phase Overview

In this training phase, Family Navigators will continue developing their understanding of school and community culture, as they solidify their role within the school teams and school district at large. They will collaborate with internal and external team members to assess the needs of and deliver support to clients, families, and the community.

Phase Objectives

  5. Continue communicating with families and the community to identify additional areas of need

  7. Begin honorable closing with families receiving support throughout the school year

STEP 1

Role Development

Continue learning about the Family Navigator’s role in transforming schools into Centers of Wellness, emphasizing comprehensive student health.

    • Continue to navigate Family Navigator role and incorporate Diversity, Equity, Inclusivity, and Belonging framework into my practices and professional engagements

    • Apply awareness of DEIB framework in work with students, families, school staff, and program team members

    • Acknowledge the value of introspection and self-awareness when circumstances or conversations bring discomfort

    • Attend and participate in regional team meetings, SCOW program meetings, professional development and training, and events within school community

    • Attend trainings and collaboratives relevant to role, including those provided by community partners and local/statewide entities

STEP 2

Providing Support Services

Learn to support clients, families, and school communities through the end of the school year, including planning for the closure of support services, ensuring accurate documentation, and offering peer support to colleagues and staff.

    • Work collaboratively with Clinicians to complete registration packets

    • Listen and provide confidential support to students and families, including during school meetings

    • Collaborate with clinicians and site staff to identify families in need of support or resources in the community, specific to holiday outreach programs and services

    • Continue to actively participate in COST

    • Engage/outreach with community/families alongside Clinicians

    • Begin discussing and planning for honorable closing with Clinicians to identify clients who need support with resources after spring break (contingent on district calendar)

    • Begin discussing and planning honorable closing with any clients/families that have received Family Navigator supports throughout the school year

    • Ensure all documentation for support services provided are complete in Teams and that the referral list is updated

    • Provide peer support to colleagues in one-on-one and group settings

STEP 3

School and Community Navigation

Continue fostering relationships with school, district, and community partners, staying informed about key events and opportunities for outreach and networking. Develop connections with various county departments and community resources, and engage in cultural and community events.

    • Address discrepancies between school calendar and program calendar with Coordinator, and develop work plan for calendar conflicts

    • Participate in school and community engagement, cultural recognition, and awareness events
